Getting Started with Walking for Wellbeing

Hello! We’re really looking forward to having you on the walks. Often the hardest thing with getting more active is taking the first step, so I hope you are able to show yourself some real appreciation for making the time to fill out this form today. The purpose of this survey is to get to know you a bit better, take down some necessary safety information, and find out a bit more about how active you are and how you’re feeling right now, so we can measure your progress with physical activity and see if that has any effect on your overall wellbeing. If you like, we can share your score with you, as you might find it helpful to see how far you’ve come in a few months when you have started coming on the walks. This survey has three sections, and should take about fifteen minutes to complete. Thank you so much in advance, and do get in touch with us if you have any questions!

Your privacy is important to Oxfordshire Mind, and we are committed to letting you know how we use your personal data responsibly. You can read our full confidentiality agreement below: Oxfordshire Mind holds personal data about yourself including your name, title, telephone numbers, address or living situation, email address, date of birth, gender, marital status, information about your mental health or physical disability and details of any offending history. This is held securely on our computer systems. We usually only share your data with other agencies if it is relevant to your enquiry and with your permission. However we will share your information in certain circumstances for example: · When there is a serious risk to either yourself or others or when we are concerned about your safety · Where there is a legal requirement to share information, such as a serious crime or a child protection issue · When we are required to share information with our commissioners or other funders for monitoring purposes Documents relating to this include Oxfordshire Mind Privacy Policy and the Confidentiality Waiver. These can be found on our website if you would like to see the full statements or I can send you one out in the post. Copies of these should also be available at your local Mind group We also share data with our funders, Sport England and CFE who help manage their data. You can read full details of how they use your data below: Sport England has given money to the Walking for Wellbeing Project to pay for the activity that you are taking part in. Sport England has asked CFE Research to look at how well it is working. To help the evaluation team do this we would like to collect some information about you. This is important because if we know what impact Walking for Wellbeing is having we can use that information to show why and how other projects should be funded in the future. That means that more people can get to benefit from opportunities such as this. The Walking for Wellbeing Project will collect data about you and your involvement in this project on behalf of CFE Research (who are conducting the research for Sport England) by asking you to complete this survey. Who will see my information? Your information will be shared with CFE Research and Sport England. No information that could lead to your identification will be disclosed in reports or shared with any other third party. Information you provide in this survey will be held for no more than twelve months following the end of the evaluation in September 2022. A copy of the full privacy notice is available from the Walking for Wellbeing Project. How will my information be kept safe? Appropriate safeguards are in place at the Walking for Wellbeing Project, CFE Research and Sport England to secure the collection, storage, transmission and disposal of your personal information in accordance with the General Data Protection Regulation. Projects are also bound by a Data Sharing Agreement which is monitored and enforced. What if I don’t want to complete this survey? Participation is entirely voluntary. If you elect not to participate, the Walking for Wellbeing Project will still ask you if they can collect some very basic personal information, such as ethnicity and gender. This is simply to demonstrate the number and characteristics of people who participate at the Walking for Wellbeing Project. You do not have to provide this information if you don’t want to. What if I change my mind? You can withdraw your consent at any time without giving a reason; this means that if you change your mind we will remove any personal information held about you within 28 days. To do this please contact a representative at the Walking for Wellbeing Project.
