We believe looking after your mental health is just as important as your physical health.

Our Physical Health and Wellbeing Workers work as part of your local GP to accommodate your annual physical health check and signpost you to appropriate services to support your mental and physical wellbeing.

The worker will take your blood pressure, blood glucose and cholesterol levels through a finger prick. Then they will discuss your lifestyle including medicine management, alcohol consumption, smoking, substance misuse and weight management.

The workers will send you a letter with the signposting details and call you after 3 weeks and then 3 months to check in to see how you are getting on.

This service is available to anyone who is registered to a Wokingham Catchment GP Practice and on the Severe Mental Illness register.
